INTERVENTION: Active treatment: ceftriaxon 80 mg/kg + Dexamethasone 0.15 mg/kg QID for 4 days Control group: ceftriaxon 80 mg/kg Intervention 1: Control group: ceftriaxon 80 mg/kg. Intervention 2: Active treatment: ceftriaxon 80 mg/kg + Dexamethasone 0.15 mg/kg QID for 4 days. Treatment ‐ Drugs CONDITION: Acute pyelonephritis. ; Acute tubulo‐interstitial nephritis Acute tubulo‐interstitial nephritis PRIMARY OUTCOME: Renal scaring. Timepoint: 4 month. Method of measurement: DMSA. Urine cytokine. Timepoint: 4 days. Method of measurement: immunoassay. INCLUSION CRITERIA: INCLUSION CRITERIA: fever, positive urine culture by supra pubic or catheter Exclusion criteria: Urinary tract abnormality, Renal Scaring, recent antibiotic usage
