Woman 32 years, multiple sclerosis treated with Rituximab in 2007 and in January 2009. She has Hashimoto's tiroiditis, Sjögren disease, positive Scl-70 and ulcerative colitis. Mother with antiphospholipid syndrome, psoriatic arthritis and Hashimoto's tiroiditis. In 18 months (2010 to 2012) had 10 sepsis, mostly from of urinary origin? She had 3 sinusitis per year since the last 3 years. Immunologic labs (June 2012): IgG 445 mg/dL, normal IgM and IgA. Anti pneumococcal antibodies before polysaccharide vaccine: 10 serotypes below the limit; six months post vaccination: 3/10 serotypes antibodies over the limit. CD3, CD4, CD8, CD19 and CD56 within normal range? Naive B lymphocytes CD19+IgD+ CD27-: 82 % (nr: 39-84). Memory B lymphocytes without isotypic switch CD19+IgD+CD27+: 1 %(nr:3-20). Memory B lymphocytes with isotypic switch CD19+IgD-CD27+:3 % (nr: 6-35). Thorax scan: subcentimeter pulmonary nodules at lung bases. She was diagnosed as Common Variable Immunodeficiency (CVID) and is receiving IGIV monthly since the last 7 months, period in which she has not have sinusitis nor sepsis. Are her clinical and immunologic manifestations secondary to rituximab received? Has she a CVID manifested with multiple autoimmune diseases, first and with recurrent infections later?.
