Chest Radiographic Pattern in HIV-Patient Related with Immune Level at Pathumtanee Hospital

Objective: The study of chest film pattern in HIV–patient with pulmonary tuberculosis correlation with immune level (CD4).Medthod: Cross sectional analytic study of HIV-patient with pulmonary tuberculosis and CD4 level from October 2006 - September 2008. Divided patient into 2 group by CD4, CD4 \< 200/mm³ and CD4 \> 200/mm³. Review film chest by radiologist and divided in 6 pattern such as Lymphadenopathy, Cavities, Infiltration, Interstitial disease, pleural effusion, pulmonary nodules. Each film may be have more than one chest pattern. Analysis with statistic p value, if \< 0.05 have significant value.Results: The forty-five patients were reviewed, male more than female and average age about 32 years old. CD4 level average 85.27/mm³, CD4 \< 200/mm³ 37 patients and CD4 \> 200/mm³ 8 patients. In CD4 \< 200/mm³ showed prominent lymphadenopathy pattern with P value 0.39 and in CD4 \> 200/mm³ showed prominent cavities pattern with p value 0.035. The rest of 4 chest pattern shows no significant change in both group.Conclusion: In group of low immune level showed lymphadenopathy pattern and in high immune level showed cavities pattern.Recommendation: In patient diagnosis of pulmonary tuberculosis if chest film shows prominent lymphadenopathy and no cavitation should be evaluate HIV-titer level too.
