Introduction: the Multiple Sclerosis Quality of Life-54 (MSQoL-54) questionnaire has been widely used to measure quality of life in people with this condition. Although it has been validated in several countries, there is no adapted version for Colombia. Objective: to adapt and validate the Multiple Sclerosis Quality of Life-54 questionnaire in people diagnosed with multiple sclerosis in Colombia. Materials and methods: a methodological study of psychometric validation, aimed at cross-cultural adaptation and evaluation of the psychometric properties of the MSQoL-54 in Colombia, developed according to the guidelines of the International Test Commission, was conducted between 2024 and 2025. Results: the content validity index was > 0.78 for all items, with Fleiss's Kappa = 0.841. Sampling adequacy was optimal (Bartlett: χ² = 12.148; df = 1.431; p < ,001; KMO = 0.943). The EFA showed 8 factors with loadings of 0.416–0.942. The CFA showed an adequate fit (CFI = 0.851; TLI = 0.839; RMSEA = 0.0767). The overall reliability was high α = 0.883, with values per dimension between 0.858–0.955 and McDonald's Omega (0.858–0.956). Conclusions: the MSQoL-54-CO has been shown to be a valid and reliable instrument for assessing quality of life in people with MS in Colombia. (AU)
