Effect of two different suturing techniques at distal of lower second molar on postoperative soft tissue swelling following surgical removal of lower third molar

INTERVENTION: Experimental Procedure/Surgery,Active Comparator Procedure/Surgery Mesially positioned simple interrupted suture: the needle is inserted at mid‐buccal flap then followed by mid‐lingual flap of the lower second molar, creating a suture knot at the buccal flap that encircles the distocervical area of lower second molar. ,Simple interrupted suture: the needle is inserted at the distal area of both the buccal and lingual flaps adjacent to the lower second molar and place the knot at the buccal flap. Mesially positioned simple interrupted sutures (MIS) ,Conventional simple interrupted sutures (SIS) CONDITION: Postoperative swelling after third molar surgery ; Third Molar; Tooth, Impacted; Sutures; Postoperative Complications Third Molar; Tooth, Impacted; Sutures; Postoperative Complications PRIMARY OUTCOME: soft tissue swelling at the distal aspect of the lower second molar Before surgery, immediate after surgery, 2 days, 7 days and 1 month after surgery vertical distance from the gingival margin at the distobuccal line angle to a fixed reference point over the occlusal plane level of the lower second molar SECONDARY OUTCOME: facial swelling Before surgery, 2 days and 7 days Facial swelling was measured in horizontal, vertical, and oblique axes using a tape while seated: mouth to ear, eye to mandible angle, and mouth to mandibular angle, respectively.,patient satisfaction 2 days and 7 days after surgery Patient reported outcome using a VAS INCLUSION CRITERIA: 1. Male or female patients aged 18 to 30 years with bilaterally impacted lower third molars exhibiting similar eruption patterns and angulations, classified as Class I or II and Position A, B, or C according to panoramic radiographic evaluation. 2. Undergoing simultaneous surgical removal of both left and right lower third molars under local anesthesia. 3. Presence of both lower second molars in the oral cavity on the left and right sides. 4. No signs of inflammation or infection associated with the lower third molars and no use of antibiotics or anti‐inflammatory medications within at least one month prior to surgery. 5. Absence of severe systemic diseases that would contraindicate or require caution for surgery, and absence of systemic conditions that may impair wound healing.
