INTERVENTION: High thoracic epidural anesthesia with local anesthetics CONDITION: Thoracic epidural anesthesia (TEA) ; ; PRIMARY OUTCOME: Combined endpoint of 30 days all cause mortality, myocardial infarction, stroke or acute renal failure requiring dialysis SECONDARY OUTCOME: Separate components of the primary endpoint, e.g.: 30 days all cause mortality, myocardial infarction, stroke or acute renal failure requiring dialysis INCLUSION CRITERIA: Patients > 18 year, scheduled for elective coronary artery surgery without heart lung machine. (Off pump CABG, OPCAB)
