Objective: The objective of this video is present the surgical technique of a robotic radical trachelectomy with an abdominally placed vaginal cerclage for containment of a cervical tumor. Description: Since publication of the LACC trial there has been skepticism on the utilization of minimally invasive techniques on the management of early-stage cervical cancer. This video highlights the technique of a robotic trachelectomy and abdominally placed vaginal cerclage for containment of a cervical tumor for a 32-year-old gravida 0 who presented with 1B1 squamous cell carcinoma of the cervix. Specifically, the video highlights the development of the avascular planes, ureterolysis, skeletonization of the uterine artery, creation of vaginal cerclage for containment of the cervical tumor, and utero-vaginal anastomosis. With each of the above steps the video focuses on relevant anatomy and discusses important surgical tips for success. Conclusion: Minimally invasive techniques can be utilized for management of early cervical cancers. This video highlights specific steps taken to minimize the risk of tumor spread to the peritoneal cavity by foregoing the use of a uterine manipulator and placing a vaginal cerclage for cancer specimen containment.
