INTERVENTION: This study is a randomised controlled trial (RCT) evaluating the effects of Transcendental Meditation (TM) on mental health and workplace absenteeism among ambulance service personnel. The RCT will target 350 ambulance workers from the London Ambulance Service (LAS), the Yorkshire Ambulance Service (YAS), the South East Coast Ambulance Service (SECAmb) and the West Midlands Ambulance Service (WMAS). Participants will be invited via email in 2026, drawn from a combined population of approximately 9000 frontline staff and call handlers in London, 7000 in Yorkshire, 3600 in the South East Coast and 4500 in the West Midlands. The selection of these four ambulance services ensures greater representativeness of the wider ambulance workforce across diverse settings. Participants will be clearly and comprehensively informed of the intervention, and only those providing informed consent will take part. The computer‐based randomisation will be stratified by Ambulance Service. Each trust will be randomly divided into two groups: one receiving the meditation intervention, and a comparison group receiving the intervention after the study concludes. A baseline survey will be conducted before the intervention to assess initial characteristics and evaluate balance across groups. The RCT design is selected because it is the gold standard for evaluating causal effects of interventions, allowing for unbiased estimation of the impact of Transcendental Meditation by controlling for confounding factors through random allocation. This ensures that observed differences in outcomes between groups can be confidently attributed to the intervention itself rather than other external factors. Recruitment Invitation to participate in the study and registration links will be shared via staff newsletters, wellbeing bulletins, intranet posts, and communications from staff wellbeing leads. Ambulance staff are invited to register their interest and select from a list of convenient dates to attend an online group TM introductory presentation led by certified TM instructors and a member of the research team. Following the presentation, each participant attends an individual online meeting with a TM instructor to assess their eligibility. Those eligible and assigned to the treatment group are invited to choose from multiple start dates and locations over a 1‐2‐month period. Before beginning instruction, participants are emailed a weblink to: Complete a consent form Complete the baseline online survey (approx. 15 minutes, using seven validated instruments). Subsequent follow‐up surveys are sent at 3, 6, and 9‐months post‐instruction. Bi‐weekly online self‐reports of sickness absence are collected for up to 9 months. Data is encrypted and managed via Jotform. Participants use coded IDs to ensure anonymity. Personal data is accessible only to the researchers and will be destroyed upon study completion. TM Intervention Group Participants allocated to the intervention group receive the standardised TM course delivered over four consecutive days, followed by si Xmonths of twice‐dail CONDITION: Neurotic, stress‐related and somatoform disorders ; Mental and Behavioural Disorders PRIMARY OUTCOME: Psychological distress measured using the General Health Questionnaire (GHQ‐12) at baseline and at 3, 6 and 9 months Stress‐related sickness absence, measured using twice‐monthly participant self‐reporting at baseline and at 3, 6 and 9 months SECONDARY OUTCOME: Health‐related quality of life, measured using the EuroQol‐5 Dimensions‐5 Levels scale (EQ‐5D‐5L), at baseline and at 3, 6 and 9 months Insomnia, measured using the Insomnia Severity Inde X(ISI), at baseline and at 3, 6 and 9 months Professional fulfilment (including burnout), measured using the Professional Fulfilment Inde X(PFI), at baseline and at 3, 6 and 9 months PTSD symptoms, measured using the Post‐traumatic Checklist for DSM‐5 (PCL‐5), at baseline and at 3, 6 and 9 months Turnover intention, measured using the Turnover Intention Scale, at baseline and at 3, 6 and 9 months INCLUSION CRITERIA: Participants will be selected on a first‐come, first‐served basis, with the first 350 individuals who satisfy all inclusion criteria (listed below), meet none of the exclusion criteria, and provide consent for study participation being enrolled. Participants (in both the treatment and control group) must meet all of the following inclusion criteria to be eligible for the study. 1. Age 18 years and younger than 65: Participants must be adults legally able to provide informed consent. 2. Currently employed by any of the participating Ambulance Services. This includes all frontline ambulance staff and call handlers working in operational roles. 3. Willing and able to attend TM instruction sessions. Participants must be able to commit to the four consecutive initial instruction sessions (in‐person or blended format), as well as regular TM practice. 4. Willing to complete all required assessments and track their TM practice, as described below: 4.1. Pos
