INTERVENTION: N02.421.668 N04.452.528.460 N04.590.659 This was a randomized controlled trial, two‐arm, non‐blinding. Randomization was generated every 10 numbers using an Excel spreadsheet prior to patient inclusion in the study. According to the time of arrival (time and day of admission), and considering the inclusion criteria, patients were invited to participate in the research, and in case of acceptance, they were allocated in one of the groups previously established in the worksheet. Forty‐si X(46) patients were included, twenty‐three (23) randomized to the control group and twenty‐three (23) to the intervention group. The intervention consisted of Pharmaceutical Care for patients randomized to the intervention group. Clinical pharmacist services that were offered in the context of Pharmaceutical Care included pharmacotherapeutic follow‐up (including during the consultation the application of the Medtake test and Brief Medication Questionnaire questionnaires, determination of the Charlson Comorbidity Inde Xand pharmacist intervention when Drug‐Related Problem was identified), medication reconciliation and medication review. The pharmacist was not involved in the care of patients in the control group and data were obtained after hospital discharge. CONDITION: Clinical Medicine N02.421.297.195 PRIMARY OUTCOME: Evaluate the frequency of mortality in both groups within up to six months after the date of hospital admission on which the patient was included in the study, through medical records and/or telephone contact. The percentage of mortality will be calculated by the ratio between the number of deaths and the number of patients in each group (intervention and control). Pharmacist intervention is expected to decrease the percentage of deaths in the intervention group compared to the control group. Comparison between groups will be performed by statistical analysis, adopting a significance level of 5% and a confidence interval of 95%. SECONDARY OUTCOME: Evaluate hospital readmission in both groups within up to si Xmonths after the date of hospital admission on which the patient was included in the study, through medical records and/or telephone contact. The percentage of hospital readmission will be calculated by the ratio between the number of patients with a readmission record and the number of patients in each group (intervention and control). Pharmacist intervention is expected to decrease hospital readmission in the intervention group compared to the control group. Comparison between groups will be performed by statistical analysis, adopting a significance level of 5% and a confidence interval of 95%. Evaluate the length of hospital stay (days) in both groups within up to si Xmonths after the date of hospital admission on which the patient was included in the study, through medical records and/or telephone contact. The calculation will be performed by the ratio between the number of days of hospitalization and the number of patients in each group (intervention and control). Comparison between groups will be performed by statistical analysis, adopting a significance level of 5% and a confidence interval of 95%. INCLUSION CRITERIA: 18 years of age or older; both genders; hospital admission carried out by the medical clinic team; admission between 12pm on Sunday and 12pm on Thursday; patient not coming from the clinical stabilisation room, polytrauma, COVID bed and respiratory or contact isolation; not being in contact isolation; stay in the sector for at least 24 hours after admission; admission has not occurred more than 48 hours ago; registration in the medical record of at least one medication for continuous use on admission; not being described in the medical record that the patient will be discharged, has an accepted bed or is being requested a transfer bed to another sector other than the internal medicine ward; possibility of collecting information due to the ability to communicate and remember the medications for continuous use, the presence of the list or packaging of medication or the presence of a caregiver to acquire the information; not being described in medical records that ther
