Comparison of the effect of 0.5 percent levobupivacaine with dexmedetomidine for post operative pain relief in urology surgeries by means of two techniques that is peri incisional infiltration and intraperitoneal instillation versus quadratus lumborum block.

INTERVENTION: Intervention1: Intraperitoneal instillation and Periincisional infiltration: 15 ml of 0.5% levobupivacaine diluted with normal saline to 30 ml + dexmedetomidine 1mcg/kg made to a total volume of 31 ml for intraperitoneal instillation, 4.5 ml of 0.5% levobupivacaine diluted with normal saline to 9 ml for incisional infiltration. Control Intervention1: Quadratus Lumborum Block and Periincisional infiltration: 15 ml of 0.5% levobupivacaine diluted with normal saline to 30 ml + dexmedetomidine 1mcg/kg made to a total volume of 31 ml for quadratus lumborum block group , 4.5 ml of 0.5% levobupivacaine diluted with normal saline to 9 ml for incisional infiltration. CONDITION: Health Condition 1: N00‐N99‐ Diseases of the genitourinary system Health Condition 2: N288‐ Other specified disorders of kidney and ureter Health Condition 3: N288‐ Other specified disorders of kidney and ureter Health Condition 4: N288‐ Other specified disorders of kidney and ureter PRIMARY OUTCOME: The primary outcome of the study is to assess the post operative pain as both static and dynamic component in 2nd hour in PACU area using VAS Score that is Visual Analogue ScaleTimepoint: At second hour SECONDARY OUTCOME: To compare incidence of PONV using LIKERT Scale, the incidence of shoulder pain using Binary scale, to assess the post operative pain score at 6th , 12th and 24th hour after shifting the patient to PACU using VAS Score,to assess the duration of analgesia in hours that is the time taken to receive first dose of rescue analgesia between both the study groups and to assess total consumption of rescue analgesia in miligram in first 24 hours between both the study groups.Timepoint: At sixth, twelve and twenty four hour INCLUSION CRITERIA: American society of anaesthesiologists 1 and 2 patients who are undergoing laparoscopic urological surgeries like simple nephrectomy and pyeloplasty
