Current advance care planning (ACP) tabletop games mostly focus on exploring \"personal values.\" While a few explore care preferences through \"hypothetical scenarios,\" they are not disease-oriented, which can create discrepancies between the scenarios or medical treatments and the patient\'s actual condition and may fail to capture the patient\'s true preferences. A systematic review found that disease-oriented ACP interventions effectively capture patients\' and their families\' preferences for medical treatments because they closely reflect the patient\'s actual disease progression. However, no disease-oriented scenario games are currently available. Therefore, this study, using heart failure as an example, aims to develop and evaluate the impact of disease-oriented scenario games on older adults and family caregivers in ACP.
