INTERVENTION: Intervention1: Modified constraint induced movement therapy with itch stimulation: Modified Constraint‐Induced Movement Therapy Duration: 30‐minute sessions, 5 days per week, for 12 weeks. Procedure: Constraint: The unaffected upper limb is constrained (e.g., by a splint or mitt) Co treatment‐ Itch induced by histamine skin prick on unaffected arm twice a week. Training: The affected upper limb is intensively trained through shaping and task‐oriented practice (such as activities involving reaching, grasping, and manipulation). Motor and Cognitive Training: Sessions will integrate cognitive strategies to enhance motor learning and neuroplasticity. Group 2 Transcranial Direct Current Stimulation Duration: 20‐minute sessions, 5 days per week, for 12 weeks. Control Intervention1: Transcranial direct current stimulation with Functional Electrial Stimulation: Transcranial Direct Current Stimulation Duration: 20‐minute sessions, 5 days per week, for 12 weeks. Procedure: Anodal Stimulation: 2mA over the motor corte XC3 or C4, targeting the affected hemisphere. Co‐Treatment: Functional electrical stimulation to finger extensors group muscles and upper limb rehabilitation exercises will be conducted during the tDCS sessions to facilitate motor learning and cortical reorganization. Frequency:20 50 Hz, Pulse Width:200 400 s Intensity Waveform Biphasic, Duty Cycle: On: Off ratio 1:3 Ramp Time:1 2 seconds for both ramp‐up and ramp‐down Session Duration: 30minutes per session/5days per week CONDITION: Health Condition 1: G819‐ Hemiplegia, unspecified PRIMARY OUTCOME: Fugl‐Meyer Assessment for Upper Extremity scaleTimepoint: Baseline and 8 weeks SECONDARY OUTCOME: Action Research Arm Test: Assesses upper limb function through tasks that mimic daily life activities. ; ; Stroke Impact Scale: Measures stroke‐related changes in quality of life. ; ; Modified Tardieu scale: Assess SpasticityTimepoint: Baseline to 12 weeks INCLUSION CRITERIA: Age 40‐65 years Both males and females No significant cognitive impairment MMSE less than or equal to 24. Spasticity till grade 1 to grade 3 Post‐stroke patients MCA territory ischemic stroke, 0‐3‐month post‐stroke. No recent past 3 months use of anti‐spasticity medications, e.g., baclofen, botulinum toxin. No fixed contractures or severe joint deformities that limit passive range of motion.
